We have a permission/waiver form for going on events & doing activities, but not for mentoring. But it does sound like a wise idea to at the very least make sure parents know what the intentions are for a mentoring relationship so that they can opt their child out if they would like to. As far as any legal ramifications where a waiver might be helpful, though, I don't have any knowledge there. You might try contacting whoever your church's insurance company is or checking their website. Our insurance company has a lot of great resources for waivers including samples/templates. Or you might be able to ask your rep for advice or have him/her look over whatever you come up with.
