The last two churches I have been at have had a prayer room / chapel but they weren't youth specific either.

Our church has a prayer room, but it's not specific for just teens. Adults use it every Sunday and it's staffed with "prayer partners" who will pray with you if you want.

Maybe you could include a logbook where kids can write prayer requests and then update it when the prayer is answered.

Maybe a CD player with worship music.

Low lighting, perhaps.

Soft carpet for kneeling.

Bibles, of course.

Topical index of verses for every situation and issue.

I've never had a separate room for prayer, but it sounds like an awesome idea. Our church has a prayer chapel that is open for everyone. We have comfortable chairs, soft lighting, candles, bibles, everything to give it a comfortable feel.
