A couple things I like to do...

The would you rather... (questions)
-lose an arm or a leg?
-have one good friend or 5 alright friends?
-be hurt or hurt someone else?

They often open up good conversation if you ask "Why?"

But the real key is the "Relationship" go to their games, ask them what they are up to on the weekends, who's happening at school, what they are struggling with...

Wow, there are just so many! Before I offer up some question ideas though I'd like to offer up a tip: have food. It really helps both parties to feel more comfortable talking & sharing when there is something to eat.
So, questions! A good place to start if you're new & really don't know your kids very well yet it to start with the basics: grade, school, siblings, participation in school activities (band, sports teams, etc.). The three M's are also good: music, movies, & munchies.

It will be more difficult to ask pre-scripted questions that will help you discover where each student is standing when it comes to spiritual maturity. In fact I suggest you don't do that, because they'll probably be able to tell. (At least, I know mine would!)
Instead of going into these interviews hoping to gauge spiritual maturity in one sitting plan on getting the student's contact information and letting them know you would love to spend more time with them in the future just hanging out. That's the best way to achieve the goal you listed. These interviews will just be a beginning.

We did this as well. We called them tee/tea times. If it was a guy we were talking to, at the end he would get to try and putt a golf ball into a fake hole.whichever person got like the best out of 5 would win something. If it was a girl,then my wife would have tea and crackers or something. It really helped mellow the situation out.
