Here is how I would write it: Dear (Name) It is with great regret that I inform you of my resignation as youth leader at (Church Name) effective (Date) This role has changed my life and the terrific people. The future of the youth minsistry is secure. I prayed about this decision and believe what God has called me to do (List reasons for resigning) I want to thank you for the opportunity of working with people's most precious treasures, their children. If I can help with a replacement don't hesitate to ask. Our students deserve nothing less. Thanks for your understanding and the priviledge of serving the young people of (Church Name) Sincerely, Your name. (Best keep concise, clear, positive - don't burn any bridges)

I've only had to write one letter of resignation to a church, which I left to go to school. Obviously I included the fact that I was resigning and a very brief description of why (don't feel like you need to use the letter to justify leaving - if people have questions they can ask, but the letter's purpose doesn't need to be addressing all of that). In addition to that, though, I thanked the church for the opportunity to serve and tried to encourage the church and the youth group in moving forward. Whenever someone is leaving it can be an emotional time and people close to you can easily feel upset. So trying to highlight the positives of what God can do through the change can be helpful.
