I agree with Tim on this one. I have had to deal with this same issue. Take it to your Sr Pastor first.
Like Tim said, these situations can be very sticky, so make sure that leadership is informed asap. They will most likely have the wisdom to handle this.

I was actually just in a self-harm scenario two weeks ago, but it was with a guy. I happened to notice the marks on the arm and how he covered them up most of the time with long selves or a jacket. I went to our pastor first.

But there are times when the parents are often the cause of the self-harm. In those cases, let the church's leadership handle it. I've been there and it's a VERY sticky situation.

I agree with the previous answers. YES, the youth pastor and her father should know about this. This is to prevent her from doing anything more harmful to herself.

YES, by law you must pass this info along to the youth pastor and make sure he/she passes it along to her father.

The answer is Yes, to all of the above. Yes, the youth pastor should know. Yes, the father should be told. This girl needs some help and probably quick. Find a good grief counselor who she can talk to or another lady in the church to try to talk to her. DO NOT freak out about this. She is expressing the pain she is going through in probably the only way she can think of right now. Someone needs to reach out to this girl and help her. Spend some time with her. Give her time to grieve and deal with the death of her mother. Her father won't be able to help her right now because he is going through the same thing. The church needs to step in and really help this family.
